Registration Checklist
Under 6 Registration Packet
- 2-Page Greenhaven Soccer Application
- Volunteer Service Form (select either $50 volunteer donation or $50 refundable volunteer fee)
- US Soccer Club form (U6 only)
- Check or money order payable to “GHSC” (for all fees)
- Birth certificate (if new player)
Under 8-19 Registration Packet
- 2-Page Greenhaven Soccer Application
- Volunteer Service Form (select either $50 volunteer donation or $50 refundable volunteer fee)
- 2 copies: CYSA Form (CA Youth Soccer Assn); original signatures in BLUE INK
- Check payable to “GHSC” (for all fees)
- Birth certificate (if new player or did not play in 2012)
- A picture that may be uploaded and then have the facial area cropped to a 1×1 inch size. The original photo may be 3×5
The photo may be a 3×5, but once uploaded, the face will be cropped within 1×1 inch,
No hats/caps, sunglasses,
Player’s name & birthdate on back of picture.
